What an evaluation involves

Before treatment starts, I get a baseline echocardiogram and review your cardiac risk factors and history — this becomes the reference point every later test gets compared against. We use a newer automated analysis called strain imaging that helps identify problems before overall heart function is affected. Depending on the drugs and doses your oncologist has planned, we'll set a schedule for follow-up echocardiograms during and after treatment, timed to catch any decline in heart function early enough to act on it.

Some regimens also call for periodic blood biomarkers or EKGs between imaging visits. Throughout, I stay in direct contact with your oncology team, so any concern I find gets addressed without derailing your cancer treatment — the goal is always to protect your heart without getting in the way of treating the cancer.

Cardio-oncology is a recognized niche in our practice, built on partnering closely with oncologists at Delray Medical Center and area cancer centers rather than working in a silo. My job is to give your oncology team the clearest possible picture of your heart's tolerance for treatment, so decisions about dose and timing are made with full information, not guesswork.

I know a cancer diagnosis is already overwhelming without adding a second specialist and a second set of appointments. I try to make the cardiology piece as low-friction as possible — clear scheduling, direct communication with your oncologist, and results explained in plain terms so you're not left wondering what a test found.

Why do I need a cardiologist during cancer treatment?

Several chemotherapy drugs and chest radiation can affect the heart, sometimes months or years after treatment ends. A cardio-oncologist tracks your heart function before, during, and after treatment so any changes are caught early and managed without interrupting the cancer treatment your oncologist has planned, whenever that's possible.

Which cancer treatments affect the heart?

Anthracyclines (like doxorubicin), HER2-targeted therapies (like trastuzumab), certain immunotherapies, and chest-directed radiation are the ones I watch most closely, since they carry the best-documented risk to heart muscle function or the heart's electrical system. Not every patient on these drugs develops a problem — but the ones who do tend to do better when it's caught on a screening test rather than from symptoms.

What tests are used to monitor my heart during chemotherapy?

A baseline echocardiogram before treatment starts, then follow-up echocardiograms at intervals your oncologist and I agree on based on the specific drugs and doses involved. Some patients also need periodic EKGs or blood biomarkers that can flag early heart strain before it shows up on imaging.
